As an APS4 Planner, you will play a vital role in supporting participants by applying well-established policies, principles, and procedures to deliver high-quality planning and service outcomes. Working within a collaborative team, you will engage with internal and external stakeholders, providing advice and resolving enquiries to ensure participants receive the support they need.
✔️ Gather information from participants and planning partners to enable high-quality planning and evidence-based decision-making.
✔️ Assess and allocate funding based on policies, legislation, and best practice guidelines.
✔️ Work closely with participants to identify their current and future support needs, including informal, community, and mainstream options.
✔️ Liaise with key stakeholders, including Partners in the Community, to deliver participant services.
✔️ Resolve issues and complaints related to participant plans, ensuring positive outcomes.
✔️ Perform administrative and data entry tasks accurately and efficiently.
✔️ Contribute to key performance indicators, including planning targets.
To excel in this role, you will have:
🔹 Strong communication and interpersonal skills with the ability to engage with a diverse range of stakeholders.
🔹 Experience in service delivery, planning, or case management within a government or community-focused environment.
🔹 An understanding of the organisation and the ability to interpret policies and guidelines.
🔹 Strong problem-solving and decision-making abilities.
🔹 Attention to detail with excellent administrative and data entry skills.
