We are seeking an experienced Senior Case Manager with a background in social, mental and community support services and particularly experience working with youth within the community. You will possess exceptional communication and interpersonal skills and an ability to maintain a therapeutic relationship with participants of the Program.

Key Responsibilities include:

• Liaise with participants of the Treatment Intervention Court using effective supervision strategies to support and encourage their engagement in the program, and the development of pro-social behaviour.
• Completion of eligibility assessments and reports.
• Implementation of participants’ individual case plans in a timely, responsive and professional manner.
• Developing and maintaining close working relationships with relevant service providers to ensure participants have timely access to services, and to gather relevant information and feedback about progress.
• Keeping accurate and up-to-date case notes and writing objective and accurate reports for the Court, and accurately entering relevant information into the program database.
• Being responsible for the court liaison function during hearings.
• Contributing to the development of policies and procedures to ensure a high quality and professional liaison service is available for participants on the program.
• Working cooperatively as a member of a multidisciplinary team and contributing to a team approach to the management of participants by sharing skills and knowledge.

Ideally you will possess:

• Experience in a clinical setting working with people who have substance dependence disorders and/or mental health disorders and/or mental impairment.
• A good knowledge of contemporary evidence based interventions for substance dependence.
• A good knowledge of mental health diagnoses and mental health services in South Australia
• Ability to effectively engage with participants about their criminogenic needs.
• Ability to work independently and self-manage deadlines and other work commitments.

Qualifications:

• Appropriate tertiary bachelor/degree qualifications in human services, social sciences, sociology, criminology, health, community services and/or related field is essential.
