We are seeking an experienced Senior Case Manager with a background in social, mental and community support services and particularly experience working with youth within the community. You will possess exceptional communication and interpersonal skills and an ability to maintain a therapeutic relationship with participants of the Program.
Key Responsibilities include:
- Liaise with participants of the Treatment Intervention Court using effective supervision strategies to support and encourage their engagement in the program, and the development of pro-social behaviour.
- Completion of eligibility assessments and reports.
- Implementation of participants’ individual case plans in a timely, responsive and professional manner.
- Developing and maintaining close working relationships with relevant service providers to ensure participants have timely access to services, and to gather relevant information and feedback about progress.
- Keeping accurate and up-to-date case notes and writing objective and accurate reports for the Court, and accurately entering relevant information into the program database.
- Being responsible for the court liaison function during hearings.
- Contributing to the development of policies and procedures to ensure a high quality and professional liaison service is available for participants on the program.
- Working cooperatively as a member of a multidisciplinary team and contributing to a team approach to the management of participants by sharing skills and knowledge.
Ideally you will possess:
- Experience in a clinical setting working with people who have substance dependence disorders and/or mental health disorders and/or mental impairment.
- A good knowledge of contemporary evidence based interventions for substance dependence.
- A good knowledge of mental health diagnoses and mental health services in South Australia
- Ability to effectively engage with participants about their criminogenic needs.
- Ability to work independently and self-manage deadlines and other work commitments.
Qualifications:
- Appropriate tertiary bachelor/degree qualifications in human services, social sciences, sociology, criminology, health, community services and/or related field is essential.
